Fill out and electronically sign your waiver below.  Our insurance company requires us to obtain a waiver form signed by a parent or legal guardian of each student who attends this event, whether they are a minor or not.  Your RHS Graduate will not be allowed to attend the party unless we receive this form from you signed by you either electronically here
