Our insurance company requires us to obtain a waiver form signed by both a parent or legal guardian of each student as well as the student who attends this event, whether they are a minor or not.  Your RHS Graduate will not be allowed to attend the party unless we receive this single form signed electronically by you and by your RHS Graduate.
